I am over the age of eighteen (18) and am interested in helping Cat Haven as a volunteer foster parent.

All of my animals are current on ALL their vaccinations. I understand that Cat Haven foster animals will be vet-checked, FeLV/FIV-tested, and vaccinated (rabies and RCP or RCCP), unless they are underage for any of these procedures. However, I also understand that the history of most foster animals is unknown and Cat Haven will not be responsible for any inadvertent exposure of foster parents or their own animals to unknown disease or other medical condition. I knowingly accept this risk on behalf of myself and my own animals. I understand that Cat Haven expects foster parents to follow good hygiene practice at all times and strongly suggests keeping the foster animal isolated from other animals for a minimum period of ten days (inclusive of initial stay at the veterinarian) when the animal is first introduced into the home.

I will consistently bring my foster animal to scheduled adoption events as notified by Cat Haven. If I cannot transport my adoptable foster animal to an adoption event for any reason, I will notify Cat Haven and use my best efforts to help make alternate arrangements for my foster cat's attendance at the adoption event. I realize that I am expected to bring my foster animal to at least two adoption events each month, unless previously agreed upon arrangements have been made with one of the cat coordinators.

I understand that my foster animal must be adopted out through Cat Haven's adoption procedures (including an application and interview) at a fee of $75.00. If the foster animal is given away, lost, adopted or killed while in my care, I realize that I may be liable for the adoption fee.

I will provide a humane environment (adequate in size and free from hazards), loving attention and proper care for my foster animal. I understand that Cat Haven has the right to remove any animal from a foster home, at any time, for any reason. I will transport my foster animal to a veterinarian when necessary and will keep a record of the animal's name and Cat Haven number to be given to the veterinarian at the time of service. I will contact the appropriate Cat Haven representative prior to taking a foster animal to the veterinarian, unless the situation is an emergency.

I authorize Cat Haven to contact any person or entity named in this application to verify information given. A Cat Haven representative may visit my home before this application is approved or during the time that I am fostering an animal for Cat Haven.

I understand that Cat Haven will not be liable for any injury or damage to my person or property caused by a foster animal in my care and I agree to indemnify and hold Cat Haven harmless from any such injury or damage. As a Cat Haven volunteer, I agree to participate in training, acquaint myself with Cat Haven's policies and procedures, and to abide by them at all times. I understand that Cat Haven may discontinue my volunteer services at any time for any reason.
